1. Please arrive at the time of your appointment, we are unable to complete appointments earlier than your scheduled time. 

2. If you have long hair, we recommend that you tie it back. 

3. Do not eat, drink, chew gum, or smoke at least 20 minutes before the mask fit testing.

4. You must be clean-shaven on the day of the test. If you are not clean-shaven, you will not be tested. 

5. Do not bring your children/dependants to the appointment as they will not be permitted to sit with you for the test.

Once booked there are no refunds.  We can accommodate rescheduling up to 5 business days before your scheduled appointment.  Missed appointments for any reason are not eligible for rescheduling, you will need to book a new appointment at full cost.

Payment is due at time of booking!  Your appointment will be cancelled if payment is not made at time of booking.
